Please read the following carefully before completing the application form.

Ensure that you provide accurate information about yourself when filling this form. You will be automatically disqualified from the selection process if it is discovered that you deliberately misled the organization.

Brand ambassadors will be selected through the following criteria…

• Fill in the correct bio data and upload your best pictures
• You will receive a 6-digit PIN which you’ll keep and gives you one month validity throughout the selection process.
• Evaluation will be based on the accuracy of information in your bio-data, pictures uploaded, and participation in the SMSMates service.
• Response to text messages received from time to time would be an added advantage to selection.

AFFIRMATION

I am 18years and above.

I agree that the decision to participate is voluntary and fully mine. Consequently, SmsMates is exempted from any liability of risk(s) that may arise.

All decisions made by SmsMates concerning the selection of the Brand Ambassadors are final and I will not challenge or appeal against those decisions.

I waive any personal rights and permit the use of information supplied voluntarily by me for use by SmsMates.
